[~msahyoun] I'd revert that. I thought about adding the first two paragraphs
to PDDocument. But if we start that we'd also need to add something to e.g.
AcroForm that when creating a field you need to ensure that you also create the
appearances, when you add an annotation you'd also need .... - I think most
users of PDFBox understand that it is a low level lib.
