I did some reading of
https://www.adobe.com/content/dam/Adobe/en/devnet/acrobat/pdfs/PPKAppearances.pdf
and wrote a post to the dev mailing list... we are not to use the n1 layer at
all. Both text and image should be in the n2 layer and the n0 layer should be
empty.
