Yes - you have to upload the page somewhere since it was stripped from
the mailing-list post.
Drat, I forgot that attachments are stripped.
Anyway have a look at this :

Note that I have javascript normally controlling the first panel shown so that
when Javascript is not available in the browser that the default part of the
page being shown is to redirect the submitter to the normal page.
Making the back button working would be glad.
