Messages that have application headers have a very high overhead compared to
messages that don't have application headers.
There are multiple causes of this problem:
* Messages are copied unnecessarily in the client code
* Encoding/decoding FieldTables is inefficient
* Encoding/decoding FieldTables is done unnecessarily
* Copying FieldTables is inefficient
* The mapping of Variants to the current underlying FieldTable implementation
may be inefficient
