These changes (which should be in 0.16) improve the 10 headers case from 55,000
Msg/s to about 80,000 Msg/s.
They address some inefficiencies in the Field Table implementation by avoiding
most unnecessary decode/encodes.
They remove the most obvious unnecessary broker copies of FieldTables (in the
Exchange interface)
There is still some way to go though.
