This is a little hard to reproduce on a developer system. It seems that you
need to have the system under some load for it to happen.
This indicates it is some sort of race.
It was eventually reproduced using Debug builds by running the C++ example
broker in one window and then running
{{start /b examples\cpp\Debug\multithreaded_client_flow_control.exe localhost
examples 10 2}}
multiple times in quick succession, after launching maybe 10 instances one of
the processes had hung, and could have a debugger attached.
({{start /b}} starts a process in the background without opening a new console
window)
