- does a host connecting over the internet as localhost feel legitimate?
- does the connecting "localhost" resolve to 127.0.0.1?
- non FQDN in HELO?

imo, enough reasons to score 4.0 or higher

shouldn't your jailed hosts file include:

127.0.0.1       localhost.localdomain localhost
192.168.0.55  whateverhostname.dom.tld  whateverhostname
