Maybe this could be used as part of a larger overall anti-spam solution
which includes Capchas on new user account creation? Or.. something else?

Captcha would help too, I think you are on a good way.

Here is an example [1] of MediaWiki with FancyCaptcha addon [2].
FancyCaptcha shows an additional image in the form of account creation.

Although [2] says use of a word list, I did not use any word list, but
used random numbers of 8 digits since the main target users of the wiki
site are Asian people who do not usually use any English letters.

FancyCaptcha also asks users to enter a text rendered in an image upon
user's submitting a revised Wiki page if its content has any new URLs.

As Masahisa Kamataki, an author of the book about MediaWiki [3] had
suggested installation of Captcha, I have installed it in the Wiki.
Thanks to his suggestion, I have never experienced any spammer account
or spam page creation so far.
