I've uploaded a draft of the withdrawal request in both OpenOffice and the 
generated pdf form.
We might want to make it easier to sign the document "automatically" like we do 
for the Conflict of Interest affirmations, but it might be better to make it 
non-trivial to do this. 
The form has a date entry field that is simply text. I'm not sure whether we 
can change it so that only valid dates are accepted. It might not matter anyway.
