First option: you use something like Dreamweaver to make the HTML code of the form + notification and you copy/paste it in a Jahia big text.
Second option: there is an "hidden" template in Jahia 4.0.6+ default corporate templates (genericform.jsp) you can manually add. It allows you to insert a (hardcoded) form on a page + automatically send a notification
Third option: help us develop a Form Builder for the next release ;-)

We have many customers asking for a "Form Editor" functionality. What I think they want is a WYSIWYG-editor where they can draw a html form and then publish the form on a page. They also require to be able to take care of the posted answers.
