Enabling/disabling could be done with a contextual menu or via the
action button at the bottom of the list. Disabled accounts could be
separated with a 'Disabled Accounts' header. Do you really use that
checkbox to enable/disable all the time? I certainly don't. When I do,
it certainly wouldn't be a big deal to right click instead of the box.

I actually kind of like this one. I could see it working.

As far as the issue of long names, you can show the full name with a
tooltip if it needs to be truncated because of length.
