I normally use two GND vias on every QFP pin and at least one for each BGA
pin. However I haven't had time to test Milkymist/LM32 on the board.

What hapenens if you run a half the clock? Perhaps some timing problem?

Any hidden/forgotten clock domain crossings were you wait for that signal in 
several
places and it arrives in time in one place and too late in another and the
logic stumbles on it?

Any chipscope debugging?
