I guess this is right, since NETWIN only avoids for the user entering
some random PIN and since there is nothing about how to make the
settings as primary in the OTA OMA specs.

Well, I was expecting an answer kind of "we are having Vodafone here
which sells customized, but well known brands phones where settings
are burned into phones with no possibility to change or modify them"
or something like that :)

Anyway, the problem we're discussing seems to have more general
affect, for example, mostly, users also have to receive settings for
their phone's media player to watch some video stream, but they still
have to mark them as active.

I was actually started to think about some reversed solution, where
the address (in most cases this is URL) would define (in GGSN or
somewhere else) how to reach that address. This is in fact not
possible for sending MMS, since some "home url" must be defined in the
phone itself.

I'm also thinking why guys from standards organizations like ETSI
didn't define some EF in SIM for MMS settings. IMHO this kind of
solution would have worked perfectly as it works for SMS parameters
defined in EF_SMSP under DF_TELECOM.
