Here is how I resolved this.

1) Install Visual Studio Express 2008.  Recompile NYTProf from trunk.
Same issue.
2) Install pre-built from http://trouchelle.com/ppm/Devel-NYTProf.ppd.
Same issue.
3) Install Perl 5.8.9.  Success.
