I didn't know about SmartOS on BHyve; I'll give it a shot if I can't
get OmniOS working.

KVM on stock Ubuntu Server 14.04.03, running on dual Xeon E5504. It
looks like the crash is related to the NIC.  I was using SRIOV with an
Intel 82576 NIC.  But now I see that if I switch to an emulated
rtl8139 NIC, I can successfully boot the OI and OmniOS stable ISOs.

It looks like the OmniOS Vagrant image is out of date.  It's based on
r151014 instead or r151016.  Do you know who to bug to get a new
Vagrant build?
