I have .htaccess ...didn't do me any good though. Some hosts won't let you use it.  I tried a few (but not all) suggestions I found over at the drupal site but none worked 100%.  My host claimed that they've normally got them set to OFF..so they switched it for me (for now..until someone else complains ;) ).  FWIW I just installed 4.3.3 on IIS locally and the default was ON...so...
 
I did see an unassigned (I believe) task to fix this at the drupal site....but can't find the page now.

I have installed drupal twice in the last two months, once two months
ago and once a few days ago, both times using cvs. Neither distro had
an .htaccess file included. FYI.
