I installed for the first time a few days ago--got whatever the latest RC is and it came with an .htaccess file. I guess bottom line is that if we include one that has whatever settings we need in it, then it should work without having to mess with php.ini.

I have installed drupal twice in the last two months, once two months ago and once a few days ago, both times using cvs. Neither distro had an .htaccess file included. FYI.
