There is no context.xml
The 'relative urls' in the web.xml is set to false
There is no context name in the server.xml
Are there any required libraries that I miss? There are no error messages in
the logs. Only when I force the index.jsp I get:

Because it starts looping the index.jsp.
I don't get it. Sometimes it works, sometimes it doesn't. Might be some library
conflicts?

They could be present in server.xml or in the context xml. The 'relative
urls' entry would normally be false or commented out, wasn't it?

That would be very odd.
Could it be that the context name in server.xml (or the context's xml)
is odd? IIRC it must be "" for ROOT and "/something" for something
else, and things become confusing if there are more or less slashes
there then 0 (for root) or 1 (otherwise).
