What is telling you that it "appears" to be deleted but it's actually not? I've just tried a couple of situation but I wasn't able to reproduce your problem. If Axapta let me delete the order and it disappears from the list then it IS deleted. Other user will not know until they refresh.

Using Axapta 3.0 I have a situation where there are times someone
will delete a Sales Order, and it appears to have been deletd, but in
actuality, the order still remains. Is it possibble if two people
have the same order open this can occur? Is anyone experiencing
anything similar?
There has been no custom code on any of the delete methods for all
applicable data sources and tables.
