Delete records in table smmPhoneParameters Table. Other thing , go to Control 
panel - Phone and Modem options , Advanced tab. Try to see if you have some 
issue there.. If it does not wrok still, please send me screen shot of records 
in the table and Advanced Tab.
   
  Do you have anything related to TAPI installed on that system? OR did you 
move data as part of upgrade,etc..
