You might like to check out my contrib for ezmlm which sets reasonable
defaults for lists. The user interface is currently primitive - it needs
to have drop-down lists or checkboxes, but it is functional. It doesn't
currently handle moderators - you'll need to add those manually as the
ezmlm user.

I'd suggest you check the qmail logs to see where the mail is going.
Also, it is critical that you do all list moderation as the user ezmlm
(if you set up a separate user, which I suggest). If the files are owned
by root, ezmlm won't be able to read them and process the mail. My contrib
does this for you.
