As an alternative solution to hacking tcpserver and rblsmtpd, you could look at qpsmtpd. No C/C++ code required, just a little Perl.

The Howto was written when I was using SME in server only mode, and is missing a crucial step in Server and Gateway mode, where you have to tell SME to open port 25. If you handle large amounts of mail, you might want to run qpsmtpd under pperl or Speedy CGI.
