This problem seems to have disappeared with a reboot into a new
kernel, although there is nothing new in the kernel to explain why.
Oh well, let sleeping dogs lie.

I did some comparisons with live TV and the same live TV signal
recored and played back (only slightly delayed) through mplayer.  The
amount of visible picture is the same, however the OSD in the top left
is still off the screen.

I suspect that there is some portion of the border of a television
broadcast signal that is not seen on a television.  I have noticed
this when I compare a television program on the television with what I
see when I record it and play it on the computer.  There is a lot more
bottom border on CNN (i.e. under the scrolling text) for instance when
you watch it on a computer than when you watch on regular television.

Anyhow, this seems to be a non-issue for the moment.  I will hack
mplayer to move the OSD down a bit.
