This has happened to us a few times.  In fact OpenSRS sends a notice saying
that the transfer was cancelled due to registry timeout, when in fact the
transfer has completed at the registry.  Basically the e-mail (or however
the registry notifies OpenSRS) is never received by OpenSRS putting the
domain in limbo.  I am still waiting on a response from OpenSRS support
(been sending a message daily about it).  The customer's domain is basically
unavailable as the name servers are incorrect... HELP OPENSRS!!!
