I really have no idea.  Either wouldn't surprise me, Loren.

But the problem is that in a matter of says, regardless of what the
cause it, the domain will be untransferable anyway.

If it is NSI's fault, you won't get them to do anything about it.
They have shown already that they will do everything possible to
hinder outbound transfers, and play stupid later.

This is why I always recommend doing it many many months in advance,
no invoice on the books, and you have plenty of time to correct any
problems that occur.

I'd say keep trying until the anniversary.  Make sure your customer
knows that he has to ack both emails, and when the expire date comes,
tell you customer they have to pay NSI.

There isn't much else you can do, or that the rest of us can do for
you except to commiserate with you and your customer.
