If you want to get both, you can use the PDF-Import-Extension. You can crea
te with
that extension hybrid files, that opens with the programm module, that was
used to
create the file, or with a pdf-reader.

I have tried and used the hybrid. Although, I found that the combination of the LibO forms file export to .pdf method to be more efficient. I then send the groups both files, in case they wish to modify the forms. The .pdf form fillable is the form they use for public use. Most groups are now aware of the OpenOffice.org suite (LIbO) and either have it or have not problem downloading it if it is going to make their lives easier.
