I would just like to endorse the following points:
- Post/postal code rather than zip (in Australia it's a postcode -
zip just sounds American)
- There are codes with alpha characters, not just numeric
- Some codes have spaces in them
- Some countries don't have codes (e.g. New Zealand, which until
recently didn't use them at all. I believe they have just reinstated
them officially but they haven't quite caught on with the general
population.

I have toyed with the idea of a single entry field for postal
addresses that are being used just for posting things. Something
along the lines of this:
http://www.codinghorror.com/blog/archives/000532.html. I think the
big stumbling block is that you often want the address for other
things too.

Richard Ishida's post (submitted earlier). It's proven very popular

The other thing you need to think about (yes, international forms
with address questions are a pain) is state/province/county/region
etc.
