Thanks so much for mentioning my humble little blog post! That's cool, I have 
to admit. 

I still can't figure out why there's a difference in creating a Visio PDF via 
File/Print/Adobe PDF vs. the Adobe PDF menu on the toolbar. Perhaps it has to 
do with what version of Acrobat you have installed, and I'm no guru of printer 
drivers. 

I should mention that for basic interactivity, File/Save as Web Page isn't bad. 
If you set up each shape as a separate hyperlink, using Sub Address as you 
mention, then you can jump between wireframe pages and emulate a user flow.

Going back to Mark's original question, though, it still only shows whatever 
annotations are on the page. However, I prefer this to extracting the 
annotations into a separate file, because you lose the context that way.

I also found this article on for the clickable PDF.  I don't have
the option to create PDF files other than File ->Print so I can't
make it work, perhaps you can.
