Not really what you're asking I know but as an example of a good retrieve password mechanism, I like the Tumblr implementation which is very simple: http://www.tumblr.com/login Tumblr uses email as username. If you want to recover your password, you enter your "account email address". Of course, this doesn't help you remember which email address you used as your username, but how many different email addresses would users be expected to have?
