We've encountered a similar situation, and it was better to include the choices with each question vs. a matrix, even though we used the same scale/choices for every question.
The problem with a matrix is page length. If people scrolled down and couldn't see the choices, they'd forget what they were. I'd expect this be worse if you have "very ___" at both ends of the scale. . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . .
