Mike, If we do assume, form is some kind of a conversation it means, modal popups are are disturbing it by definition.
IMHO, Modals should be rather used for actions (data) that are not essential part of the form filling (or other process). Jumping in into the conversation (that is for me form filling) with additional questions is not a good user experience. Having that said, I would keep depended fields inline, dynamically loaded and/or enabled. Naturally, all above is good in theory, but practically depends on the form we are talking about.
