I think iFolder would meet all those requirements. Runs on your own servers, ldap auth, create shared folders, web based and client based. it's a Novell/SuSE product, but there is an open source version: http://www.ifolder.com/ The current 3.8 version is pretty stable, but there doesn't appear to be any updates in a while.
