Check out VistaPrint

My professional musician friend uses them as well as game designer friends.

Here is the page for custom design embroidered shirts and hats you can compare
with other sources:
http://www.vistaprint.com/embroidered-activewear.aspx?txi=352&xnid=TopNav_Embroidered+Products_Apparel+and+Accessories_Home+%26+Family&xnav=TopItem

I've seen the result of a custom design Tshirt first hand (i.e., the design was
uploaded to VistaPrint) and I can say I was impressed with the price and
quality of the result.
