When it comes to accidental disasters, imagine a mainframe console complex. A couple of periscope tubes, a couple of paper terminals all sitting on something similar to big 3 wall desk. Where the drawers would be is a bunch of electorics to drive the screens, etc. Where a drawer handle would be is a recessed 5x5 panel with a bright red emergency power off (EPO) PULL button. If you weren't careful, you could actually push into that square with your knee, but it's a pull, not a push so someone did a little work to get that correct. Now the operator sit's her purse on the free "desk" space above the EPO. Long straps on the purse. Later she gets up to take a break and the whole machine room goes quiet. Oops. Next morning there is a sheet of clear plex over the hole.
