a reduced home menu typically means you've lost contact with your player and/or SqueezeCenter. Not sure of the cause, but that's what's happening.
If you can reliably reproduce this issue it'd be very helpful if you'd open a bug at http://bugs.slimdevices.com. Be as thorough as possible, including reporting what versions of SqueezeCenter and Controller you are using (on the controller, Settings->Advanced->About, then look for the number represented by rXXXX, where XXXX is the version number). Also, I doubt you need to do a factory reset when this happens-- instead just reboot the controller.
