I think that's a MySB issue. I had this issue as well and it was because after entering the MySB credentials, SqueezeCommander immediately disconnects and reconnects to MySB. Sometimes, MySB is not fast enough and still "thinks" that the client (=SqueezeCommander) is unknown. If I wait some seconds and then try to reconnect, it works. (I.e. I wait some seconds, then press on the "more arrow" next to "MySqueezeBox.com", select "disconnect" and then "connect".)
