This is a problem with the current portaudio library. It sounds like squeezeslave doesn't find the hdmi device you've defined with -o/-n and reverts to the default 'Built-in Output'
The device must be present when squeezeslave starts and must remain active the entire time squeezeslave is running. Try running squeezeslave -L with the tv off and again with the tv on. Are the lists different? I've uploaded a 'new OSX test build' (http://squeezeslave.googlecode.com/files/squeezeslave-1.2L-320M-osx.tar.gz) using the latest portaudio library for you to try.
