In my experience all sorts of different reasons. A box set of Elvis Costellos Girls Girls Girls from the 80s ripped perfectly to Mp3 ten or so years ago when I started out with LMS. I put it back on the shelf where it has been untouched for those years. Tried to re-rip to FLAC recently and no chance with any of the usual rippers we use. I had to buy another copy. I can only conclude that the CDs had chemically degraded.
