Executive summary: reprogram Xilinx.  Press and hold "1" while
unplugging and re-plugging the power.

Detailed description: today for the first time since I got my first
Squeezebox back in Halloween of last year, my speakers did not start
producing wonderful sound when I pressed the play button.  As you
describe, everything points to it playing: equalizer, track time, etc. 
I checked the optical output.  It was active.  I checked the optical
cable.  Red light coming out of the end.

Just as I was about to try the analog and headphone outputs I checked
my receiver display.  The PCM indicator wasn't on and the display read
"NO DATA" when I switched to the Squeezebox input.

This indicates that although the optical output was active the
Squeezebox wasn't sending anything.

I turned the Squeezebox off and on to no avail - but of course that
doesn't actually turn it off, it just switches between idle/active
mode.

So I reprogrammed the Xilinx on a guess.

As soon as it rebooted the PCM indicator came back on my receiver and
everything's back to normal.

It could be that a simple power-cycle would have done it.  Also
although I didn't check the analog connections I bet they were out too
with the Squeezebox in that messed-up state.  If my solution hadn't
worked, a factory reset would be next (press and hold ADD instead of
1).
