It is certainly possible to do this - in fact I am doing it now.

The main things to do to enable this are 

a) get a fixed internet IP address for your home network.  This is
easily done by signing up with a site such as dyndns.org.

b) set up ssh tunneling on your server PC so that you can access from
outside without compromising security. 

c) forward ports 9000 and 3843 on your router to the server PC internal
IP address.

d) run softsqueeze on the remote machine and set up the ssh parameters
to match your server details.

e) limit bandwidth of the streaming for the remote instance of
softsqueeze so as to avoid dropouts over a poor web connection.

It can sound a bit daunting but it is a great thing to have once you
have set it up - all your music available whilst in a hotel room for
instance, or at a friends place.
