If you do a full clear and rescan every day, my understanding is that Slimserver completely re-writes its database and forgets about all the files which existed in your Now Playing playlist. So it will be empty every day regardless of the maintain client playlist settings.
There were changes to scanning behaviour around the time of 6.5.3/6.5.4 which may have introduced this IIRC.
