First thing to check is that you've got the right iTunes file listed in
Squeezecenter for syncing purposes.  Sorry if this sounds like a dumb
suggestion, but a couple of people have been caught out by iTunes
changing the name of the .xml file (following an upgrade) and not
updating the link in SC.  Shouldn't be your issue if the file
disappears following a hard delete but worth checking which file iTunes
is using for the library first.

In iTunes/Settings is something like iTunes Reload Interval and what SC
does is check your iTunes.xml file every interval to see if it's
changed.  If it has, it reloads the library by a new and changed music
scan.  0 turns the automatic reload off and you have to rely on a
manual scan.

Do you just use the iTunes library or do you also have an entry in the
Music Library settings of SC?  What might be happening is that you
remove the track in iTunes, but it's still in your music folder and is
being picked up for that reason. 

In either case, I suspect a full "clear and rescan" scan from SC will
sort out the problem.  You can set this up to run automatically
overnight if you have the server on 24/7.
