Funny, it seems on several web pages I've encountered the only way to
bring up the menu is to hold the control key and click the physical
mouse. VO+shift+m does not work in these cases at all. Any idea why
this is? Is this something to do with the site itself, Safari, or
something else? A curious question, not a complaint.

X, the term stems from broken Windows thinking) is actually invoking
the contextual menu.
The VO equivalent is Control-Option-Shift-M.
This does point out one of the weakness associated with the VO
modifier keys and command. If VO-space bar is click (it is not),
then, since Control-click invokes the contextual mouse menu, one would
