There has been a lot of work done in this department with Window- Eyes. I've been using Vista on a PC since beta1 was released early last year. And the changes in accessibility are huge.

I heard one really isn't supposed to, according to the license agreement, at least with the home versions. I think I would probably go with the business pro version as many of the new media things are only barely accessible with Narrator, unless Vista really changed that so JFW and Window-eyes do better now.
