Most software problems are possible to isolate, though not all problems are worth isolating. However, you seem extremely dissatisfied with the state of your Mac web access, so it probably is worth isolating them.

People have different configurations. Different webpages have different markup and code. The interaction between user, screen reader, browser, and web page is complex.

We know you're using VoiceOver and Safari 3, but you haven't specified your Universal Access configuration, your problem URL, or steps to reproduce the problem once at your URL. Therefore some problems other people are experiencing aren't even necessarily the same as your problem. You have no grounds on which to conclude that "Whatever is causing it isn't isolatable."
