Although I haven't used the Mac since System 8, let me try to make an
analogy here.  

Think about learning where things are in a new room, or if you prefer,
learning how to get from one place to another.  Once you get it into memory
where things are, you don't have a problem.  I think the same applies to the
Mac.  Once with OutSpoken, and the same applies with VoiceOver, you can find
it right away.  So, for example, if someone tells you that an object is at
the right bottom of the screen, once you have learned the screen, you can
then find it right away.  

As has been stated many times before, VoiceOver and the Mac are made so that
we can work with sighted people as well.  This, in my opinion, is how it
should be.

Maybe I've missed something here...but...

While I agree it would be nice in some cases to have that info  
automatically, it is available with VO-H when on a link that doesn't  
ahve an Alt tag specified.  I can navigate this site's image links by  
using it...and once i know the location of particular images, I don't  
have to hit VO-H on every single one.
