Could it be that the files you're downloading are so small that by the time you get to the window, it's already been downloaded, therefore no longer showing up in your list?

OK, I tried looking at the downloads window. I think the problem you are having here is that they use grouping, so there are two levels of interaction due to grouping, so
you want to use the VO-keys+shift+down arrow sequence twice, then
VO-keys+right arrow and/or tab. This will let you get to the "Show in Finder" button
and the Clear button.

So, for example. I go to my Downloads window with command+option +L and hear "Downloads Window three items list". I interact with VO-keys +shift+down arrow and hear "Interact with list group" and VO-keys down arrow to one of the items I interact with VO-key+shift+down arrow and hear "interact with group four items" and the file size. At that point I stop interacting (VO-keys+shift +up arrow).

Then I VO-keys+right arrow three times first to the file name, then to the file icon button, then to the "Show in Finder Button" VO-keys space bar takes you to the Download location in a finder menu, where you can delete the file. If you didn't want to delete the file, you could have tabbed to the "Clear" button. This doesn't clear off files. It clears completely downloaded files from the list in the downloads window.
