It sounds as though you should be able to use
VO-keys shift space bar to click.  That relies on
having 

Mouse cursor tracks VoiceOver Cursor 

checked although, as Dan said, some people
have had trouble with this combination.

Some people uncheck "VoiceOver Cursor tracks Mouse
Cursor" because, in case they have a mouse attached
and accidentally move it, the VoiceOver Cursor location
will also move unpredictably.

With these settings do you still have problems
clicking?  The only instances where I've had problems
with this (on a PowerBook and also on a
recent Intel MacBook) is when the item I try to click
on is actually off the screen.  That's rarely the case.
