I'm using Tiger, Safari 3.0.4, and webkit 523.12 (that's what installed with Tiger 10.4.11. Safari is behaving very well actually , and with this version you get heading read correctly. There is no VoiceOver command to move among headings, but at least VoiceOver reads the stuff inside the heading now instead of simply saying heading and making you interact with it to read it! Big improvement!!
Command+r does in deed refresh the page.
