Yip it happens, I just hit cmd and f5 keys, and my mac, says vo on.

TK: I've noticed this on occasion as well. The majority of the
time when it happens for me is when I'm in a rush to get going. I
open the lid, start pressing VO keys and arrow keys to find out
exactly where things are and it just won't talk to me. It's like
my wife when she's upset with me, just gives me the silent
treatment. I've learned to simply press the control key a number
of times upon opening the lid to give the MacBook Pro time to
stretch and wake up. This usually works. Sometimes though,
whether by my doing or the computer's, it wakes and won't talk.
In that case, I simply press cmd-shift-q which will bring up the
"Are you Sure You Want to Log Out" dialog. I then press return,
let it log out then log right back in and everything is back to
normal. No need for the complete restart.
